There are four different phenotypes, categorised into Type A, B, C or D. Type A has the highest rate of incidence of between 44-65%, while Type D is seen least frequently with maximum incidence of 23%.
Diagnosis of the four phenotypes display different characteristics:

Mildly elevated serum insulin, atherogenic lipids and androgen levels. High incidence of hirsutism, diet plays an important role in development of obesity, metabolic syndrome common.
Polycystic ovaries, oligo-anovulation, without hyperandrogenism
Normal androgen with slightly elevated endocrine levels and lowest metabolic dysfunction. Elevated SHBG, low T3 & T4, lower LH/FSH ratio, regular menstrual cycle with intermittent irregularities.
As each Type varies in presentation and symptomology, so too do the genetic pathways involved.
